Camp / Program Refund Policy

Injury

1. If injury occurs prior to the first session, you may apply for a full refund. An injury report and Doctor's note along with the refund request should be submitted to hattricksp@outlook.com
2. If injury occurs during the off season camp/program a refund may apply prorated on the remaining sessions. An injury report and Doctor's note along with the refund request should be submitted to hattricksp@outlook.com
3. If approved, refunds will be processed as a credit to be used in a future program.

Relocation

1. Should the family of the registered player relocate directly prior to or during the session, a refund will be provided prior to session or prorated during session. Refund request along with date of departure will be submitted to hattricksp@outlook.com

Non Attendance

1. Refunds will NOT be considered should a player not attend the camp/program for which they were registered.

Cancellation

A refund may be provided, less the processing fee, should a registered player request cancellation from the program two (2) weeks prior to the first scheduled session. The processing fee of $25 will NOT be refunded except as noted above, unless the session is cancelled due to low enrollment.

Release / Waiver of Liability, Assumption of Risk, and Indemnity & Parental Consent Agreement

Must be signed before the 1st day of the clinic. IN CONSIDERATION of being permitted to participate in any way in the "Hattrick Academy" ("Activity") I, for myself, my personal representatives, assigns, heirs, and next of kin:

1. ACKNOWLEDGE, agree, and represent that I understand the nature of Soccer Activities and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the Activity.
2. FULLY UNDERSTAND THAT: (a) SOCCER ACTIVITIES INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH ("RISKS"); (b) these Risks and dangers may be caused by my own actions or inactions, the actions or inactions of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c) there may be OTHER RISKS AND SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation or that of the minor in the Activity.
3. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE Hattrick Academy (Hattrick Soccer Performance LLC), their respective administrators, directors, agents, officers, members, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owner and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS. I FURTHER AGREE that if, despite this RELEASE AND WAIVER, I or anyone on my behalf makes a claim against any of the Releasees, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim.

Minor Release

I, THE MINOR'S PARENT AND/OR LEGAL GUARDIAN, UNDERSTAND THE NATURE OF SOCCER ACTIVITIES AND THE MINOR'S EXPERIENCE AND CAPABILITIES AND BELIEVE THE MINOR TO BE QUALIFIED, IN GOOD HEALTH, AND IN PROPER PHYSICAL CONDITION TO PARTICIPATE IN SUCH ACTIVITY. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS EACH OF THE RELEASEES FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON THE MINOR'S ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS. I FURTHER AGREE THAT IF, DESPITE THIS RELEASE, I, THE MINOR, OR ANYONE ON THE MINOR'S BEHALF MAKES A CLAIM AGAINST ANY OF THE RELEASEES NAMED ABOVE, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS, LIABILITY, DAMAGE, OR COST THAT MAY INCUR AS THE RESULT OF ANY SUCH CLAIM.

In consideration of being allowed to participate:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation.
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS the Releasees, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event, WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Media Release Form

I, for myself, and/or for my child, as their parent/guardian give Hattrick Academy the rights to interview, take photographs, other images, audio, and/or audiovisual recordings of me and/or my child to be used in promotions, education, internal training, website, social media, and/or fundraising materials. This includes, but is not limited to video tapes, pamphlets and brochures.

By signing this media release, I intend to legally bind myself, my minor children, my heirs, executors and administrators. I acknowledge that Hattrick Academy shall have all the rights of copyright in and to such photographs and videotapes and may use such copyright fully.

I, for myself, and/or for my child, as their parent/guardian hereby release Hattrick Academy and its officers, agents and employees from all liability connected with the taking and use of these materials.

I for myself, and/or for my child, as their parent/guardian waive all rights, interests or claims for payment in connection with any exhibition of these materials.

If I do not agree, I must reach out to hattricksp@outlook.com before proceeding to participate.

This consent is voluntary and I, for myself, and/or for my child, as their parent/guardian give it in the interest of public information, education, the furtherance of the goals of these institutions, or other lawful purposes. I acknowledge that I have legal authority to sign this form on behalf of myself, and/or for the minor whose name is mentioned.

In consideration of being allowed to participate in the event or activity referenced above, I acknowledge, appreciate, and agree that: (1) The risk of injury from the activities involved in this program is significant; (2) I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS; (3) I willingly agree to comply with the stated and customary terms and conditions for participation; (4) I HEREBY RELEASE AND HOLD HARMLESS the Releasees WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, to the fullest extent permitted by law.
